چکیده مقاله:

Introduction: Concurrent chemoradiotherapy (CRT) is a standard treatment method for patients with carcinoma of cervix. Despite admirable therapeutic results, acute hematologic toxicity (HT) is common with this regimen. Many studies evaluate the relationship between the bone marrow dosimetric parameters and the severity of HT and result shown that the low dose of bone marrow would decrease the incidence of HT in patient undergoing concomitant CRT. In this study the dosimetric parameters of two radiotherapy plans in managing of cervix cancer has been evaluated to investigate the best plan on the way of reducing bone marrow dose.   Materials and Methods: Computed tomography scans of 18 patients with carcinoma of cervix were acquired and transferred to the 3D treatment planning system. The corePlan3D treatment planning system was used to contour all of the structures and to compute the dose distribution for plans based on collapsed cone convolution (CCC). For each patient, two different treatment plan were prepared using 15 MV photon energies. The minimum dose (Dmin), maximum dose (Dmax) and mean dose (Dmean) to target (PTV) and femoral head were compared for each plan.   Results: Result shown that our manipulated four-field plan which use anterior oblique beams seemed to deliver a lower mean dose to femoral head against conventional box fields (Dmean=66.7±3.1 cGy, 98.8±7.3 cGy, Dmax=117.5±3.0 cGy, 126.9±4.3 cGy, respectively) and deliver sufficient dose to PTV (Dmax=213.5±4.2 cGy, 194.6±2.0 cGy, respectively).   Conclusion: With suspicious planning in respect to choice of beam angles, beam weighting, and appreciation of potential exposure of normal tissues to exit dose, this study revealed that compared with conventional box fields, anterior oblique beams can achieve significant reductions in the dose to BM with adequate dose coverage to the PTV with an accurate beam arrangement

Dosimetric comparison of whole breast radiotherapy using field in field and conformal radiotherapy techniques in early stage breast cancer

Background: We aimed to compare field-in-field technique (FIF) with conformal tangential field radiotherapy (CRT) in terms of dosimetric benefits for early stage breast cancer radiotherapy. Materials and Methods: Twenty consecutive left-side breast cancer patients who underwent breast-conserving surgery were included to the study. For each patient, two different treatment plans were cr...

Dosimetric comparison of two different whole brain radiotherapy techniques in patients with brain metastases: How to decrease lens dose?

Background: Palliative whole brain radiotherapy (WBRT) has been the standard treatment for brain metastases. Ionizing radiation is known to be one of the most potent cataractogenic agents. We aimed to evaluate two different radiotherapy techniques with respect to the doses received by the organs at risk (OAR) in patients with brain metastasis who undergone WBRT. Materials and Methods: Ten conse...

Intensity-modulated radiotherapy reduces gastrointestinal toxicity in pelvic radiation therapy with moderate dose

This retrospective study was performed to evaluate and compare gastrointestinal (GI) toxicities caused by conventional radiotherapy (cRT) and intensity modulated radiotherapy (IMRT) in 136 cancer patients treated with pelvic radiotherapy (RT) with moderate radiation dose in a single institution. A matched-pair analysis of the two groups was performed; each group included 68 patients. Convention...

Potential of Proton Therapy to Reduce Acute Hematologic Toxicity in Concurrent Chemoradiation Therapy for Esophageal Cancer

PURPOSE Radiation therapy dose escalation using a simultaneous integrated boost (SIB) is predicted to improve local tumor control in esophageal cancer; however, any increase in acute hematologic toxicity (HT) could limit the predicted improvement in patient outcomes. Proton therapy has been shown to significantly reduce HT in lung cancer patients receiving concurrent chemotherapy. Therefore, we...

Comparison of conformal and intensity modulated radiation therapy techniques for treatment of pelvic tumors. Analysis of acute toxicity

BACKGROUND This retrospective analysis reports on the comparative outcome of acute gastrointestinal (GI) and genitourinary (GU) toxicities between conformal radiation therapy (CRT) and intensity modulated radiation therapy (IMRT) techniques in the treatment of patients with pelvic tumors. METHODS From January 2002 to December 2008, 69 patients with pelvic tumors underwent whole pelvic CRT and...
